Purpose |
|

The Child Service Coordination Program offers free case
management services by a social worker or public health nurse that
includes helping families find and use community resources. This
program supports families to ensure that their children reach their developmental potential. Referrals and
inquiries accepted from families, community members and health care providers. |

Monday - Friday, 8:30am-5:00pm by appointment only.

Eligibility |
|
|
Services are offered to all children, birth to age
three, who are at risk and to children, ages three to five years, with
specific special needs. |
